Cancellation + No Show Policies
CANCELLATION/NO-SHOW POLICY FOR SCHEDULED APPOINTMENTS
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting a much-needed treatment.
Conversely, the situation may arise where another patient fails to cancel, and we are unable to schedule you for a visit due to a seemingly ‘’full” appointment book.
Restoration Lab Med Spa requires that a credit card number be kept on file to reserve your scheduled service. If an appointment is not cancelled at least 24 hours in advance, you will be charged 20% of the cost of the scheduled treatment.
We understand that delays can happen. However, we must try to keep the other patients and medical staff members on time. If a patient is more than 10 minutes past their scheduled time, we have the right to reschedule the appointment. If you are more than 15 minutes late for an appointment, the appointment will be considered a No-Show and you will be charged 20% of the cost of the scheduled treatment.
We require that patients pay the account balances to zero prior to receiving further services by our practice. Patients who have questions about their bills or account may contact us at any time for clarification. If your account becomes delinquent, your treatment will be delayed until your account is brought up to date.
Restoration Lab Med Spa holds the right to send delinquent accounts to collection agencies should the account become delinquent.
CREDIT CARD SECURITY
Your security is of utmost importance. Therefore, all credit card information will be kept strictly confidential and will be stored at a secure location that is fully compliant with HIPAA laws and Payment Card Industry Data Security Standard.
