Cancellation + No Show Policies

CANCELLATION/NO-SHOW POLICY FOR SCHEDULED APPOINTMENTS

We understand that delays can happen. However, we must try to keep the other patients and medical staff members on time. If a patient is more than 10 minutes past their scheduled time, we have the right to reschedule the appointment. If you are more than 15 minutes late for an appointment, the appointment will be considered a No-Show.  If you cancel or reschedule within 24 hours of the appointment, the appointment will be considered a No-Show.  No-Show fees begin at $50 and increases to $100 for the second No-Show.  After two No-Show appointments, Restoration Lab reserves the right to require a 50% non-refundable deposit prior to any future appointments.

ACCOUNT BALANCES

We require that patients pay the account balances to zero prior to receiving further services by our practice. Patients who have questions about their bills or account may contact us at any time for clarification. If your account becomes delinquent, your treatment will be delayed until your account is brought up to date.

 

Restoration Lab Med Spa holds the right to send delinquent accounts to collection agencies should the account become delinquent.

CREDIT CARD SECURITY

Your security is of utmost importance. Therefore, all credit card information will be kept strictly confidential and will be stored at a secure location that is fully compliant with HIPAA laws and Payment Card Industry Data Security Standard.
